  • Monitoring Tool: It is a crucial biomarker for surveillance, often measured serially over time to detect changes that may indicate disease progression.

  • Thyroglobulin Antibodies (TgAb): The presence of TgAb can interfere with Tg assay results, potentially leading to falsely low or undetectable Tg levels. TgAb levels should always be measured concurrently with Tg.
  • Recent Surgical Intervention: Thyroglobulin levels can be transiently elevated for several weeks or months following thyroid surgery or biopsy due to tissue manipulation. Interpretation should consider the timing relative to these procedures.
  • TSH Levels: Tg levels are TSH-dependent. For optimal sensitivity in detecting residual disease, Tg is often measured when TSH is elevated (e.g., >30 mIU/L), either endogenously (after thyroid hormone withdrawal) or exogenously (after recombinant TSH administration).
  • Pregnancy: Thyroglobulin levels can be physiologically elevated during pregnancy, which may affect interpretation in pregnant patients with a history of thyroid cancer.

  • Standard Blood Draw: For a routine thyroglobulin blood test, no specific fasting or dietary restrictions are typically required. Patients can eat and drink normally before the blood draw.
  • Thyroid Hormone Withdrawal: If the test is part of a TSH stimulation protocol involving thyroid hormone withdrawal, patients will be instructed by their physician regarding medication cessation. This is not a dietary restriction but a medication protocol impacting metabolism.
  • Recombinant TSH Administration: If recombinant human TSH (rhTSH) is used for stimulation, there are no specific dietary restrictions associated with its administration, but patients should follow all instructions regarding injection timing.
  • Hydration: Adequate hydration is always recommended for easier blood sample collection.

  • Consistent Laboratory: For serial monitoring, it is highly recommended to have all thyroglobulin and TgAb tests performed at the same laboratory using the same assay method to ensure comparability of results.

  • Not a Screening Test: Thyroglobulin is not recommended as a screening test for initial diagnosis of thyroid cancer in individuals without a prior history of the disease.
  • False Negatives/Positives: Be aware of potential false negatives due to TgAb interference or false positives due to residual benign thyroid tissue or recent trauma.
