  • Common Causes: Falls, motor vehicle accidents (pedestrian or occupant), sports injuries, and non-accidental trauma are frequent causes in children.
  • Life-Threatening Potential: Can lead to severe respiratory distress, hemorrhage, and shock, requiring immediate medical intervention.

🤒 Associated Symptoms

  • Respiratory Distress: Rapid breathing (tachypnea), shortness of breath (dyspnea), grunting, nasal flaring, retractions, or cyanosis.
  • Chest Pain: Localized pain, tenderness to touch, or pain exacerbated by breathing or coughing.
  • Visible Injuries: Bruising, abrasions, lacerations, deformities of the chest wall, or paradoxical chest wall movement (flail chest).
  • Circulatory Compromise: Rapid heart rate (tachycardia), pale skin, cool extremities, or signs of shock.
  • Coughing/Hemoptysis: Persistent cough, especially if accompanied by blood-tinged sputum.
  • Altered Mental Status: Irritability, lethargy, or decreased level of consciousness, potentially indicating hypoxia or shock.

🛡 Crucial Precautions

  • Airway, Breathing, Circulation (ABCs): Prioritize immediate assessment and stabilization of the child's airway, breathing, and circulation.
  • Spinal Immobilization: Assume concomitant cervical spine injury in significant trauma until ruled out, and maintain C-spine precautions.
  • Do Not Remove Impaled Objects: Stabilize any impaled objects in place and do not attempt removal, as this can worsen bleeding.
  • Continuous Monitoring: Closely monitor vital signs, oxygen saturation, and respiratory effort for any deterioration.
  • Rapid Transport: Ensure prompt transport to a pediatric trauma center or facility equipped to manage severe pediatric injuries.

🍽 Dietary Directions & Restrictions

  • NPO Status: Keep the child strictly NPO (nil per os) immediately following chest trauma in anticipation of potential intubation, surgery, or diagnostic procedures.
  • Intravenous Fluids: Administer intravenous fluids as directed by medical staff to maintain hydration and support circulation, especially if signs of shock are present.
  • Gradual Reintroduction: Once stabilized and cleared by a physician, oral intake should be gradually reintroduced, starting with clear liquids and progressing as tolerated.
  • Avoid Oral Intake if Unstable: Do not offer food or drink if the child is experiencing respiratory distress, altered consciousness, or is hemodynamically unstable.

⚠️ Attendant Guidelines

  • Call Emergency Services Immediately: Dial 911 (or your local emergency number) without delay for any suspected chest trauma in a child.
  • Do Not Move the Child: Unless the child is in immediate danger (e.g., active fire), avoid moving them to prevent further injury, especially to the spine.
  • Maintain Airway: If the child is unconscious, gently open the airway using a jaw-thrust maneuver (if spinal injury is suspected) or head-tilt/chin-lift.
  • Control External Bleeding: Apply direct, firm pressure with a clean cloth to any visible external bleeding on the chest.
  • Reassure and Comfort: Keep the child as calm and still as possible, offering reassurance while awaiting emergency medical personnel.

  • Primary and Secondary Survey: Conduct a thorough primary survey (ABCDE) to identify and manage life-threatening injuries, followed by a detailed secondary survey.
